Our system is very general and not alot of paper work but we do require some paper work. To set up an account please see below. To be considered for Purchase Order Credit Please Print the application and Terms and Conditions and fax it to us at 775-361-9170 or you can send it to: Haga's Party Options Attn: Credit Department 210 N. Market St Hoopeston, IL 60942 We also need a copy of your Resale License, EIN number issued by your state. Not just the number but an actual letter or document from your state giving that information. We will contact you after the application is received within a week on whether your credit has been approved. |
